Have you ever seen a butterfly on a frog's head?

“Have you ever...?” is a question in the Present Perfect tense.

This grammatical tense is a hybrid – not 100% past and not 100% present but a mixture of the two, or in-between. “Have you ever...?” i.e. from the moment you came into this world until now – the whole time span, the ribbon of your precious life...

Grammatically speaking, to make the question from the entry title, you need:
the verb “have” + a person (all except he/she/it) + ever+ the Past Participle (-ed form of regular verbs or (III) form of irregular verbs).

Pytanie z tytułu wpisu “Have you ever...?” jest w czasie Present Perfect. Jest to hybryda – ani czas 
przeszły, ani teraźniejszy, ale połączenie, coś pomiędzy przeszłością a teraźniejszością.

Pytanie oznacza: „Czy ty kiedykolwiek...? Kiedykolwiek, czyli od momentu, kiedy pojawiłeś się na świecie do momentu, w którym czytasz - lub słyszysz – te słowa.

Technicznie rzecz biorąc, pytanie składa się z następujących części:
Have   +   osoba oprócz on/ona/ono/to  +  ever  +   czasownik regularny z końcówką –ed lub forma Past Participle (III kolumna w tabelce) czasownika nieregularnego .
